Child Heath Plus

Child Health Plus is New York State’s medical insurance program serving uninsured children under age 19 in families whose household income exceeds Medicaid income eligibility.

The insurance is provided free or at a low cost. Coverage includes well-child check-ups, immunizations, prescriptions, doctor’s visits, x-rays and lab work, outpatient mental health, inpatient hospital care, emergency medical care, and other services.

The application is a simple one page form requiring proof of the child’s age, household income, and residency. For information or assistance contact your school nurse or call [in the Rochester area 585-546-3559.

Family Health Plus

Family Health Plus is a public health insurance program for adults who are aged 19 to 64 who have income too high to qualify for Medicaid. Family Health Plus is available to single adults, couples without children, and parents who are residents of New York State and are United States citizens or fall under one of many immigration categories.
