PHYSICALS
Please note that as of 7/1/18, the NYS physical examination requirements have changed. All students entering the 9th or 11th grade as well as all new entrants must, by law, have a physical exam that was performed within the year of starting the 9th or 11th grade or entering the new school district. A copy of that exam must be on file in the nurse's office. Please arrange to have your student seen by their health care provider if they have not yet had a physical and he or she will be starting 9th or 11th grade. Your child's provider will also evaluate and provide necessary immunizations at that time.
IMMUNIZATIONS
As of September 1, 2016, all 7th and 12th grade students MUST HAVE Meningococcal vaccines (shots) in order to attend school.
NYS law requires each parent or guardian provide the school with an immunization (shot) record from a health care provider, health department, or official immunization record from the child's previous school that shows their child has received, or is in the process of receiving the required vaccines (shots) in order to attend school. The date each immunization was given must be included in the record.
- Students in Grade 7 will require proof of 1 dose of meningococcal vaccine.
- Students in Grade 12 will require either:
- 2 doses of meningococcal vaccine with the booster dose given on or after age 16
- 1 dose if your child’s first dose was given on or after age 16.
Please contact your health care provider to make sure your child has what they need to attend school this fall. Bring the immunization record to the School Health Office where it will be copied and returned to you if you wish or feel free to fax to 315-524-1088.
NOTE: If you child does not have the proper immunizations they will not be able to attend school.
